Replace the a tag with a button:
<a href="Online_Application" ><button class="btn btn-info btn-md col-md-sm-2">YES </button> </a>
With submit button:
<button type="submit" class="btn btn-info btn-md col-md-sm-2">YES </button>
And set the action and target in form tag:
<form target="_blank" action="Online_Application">
